These forms give us the information we need to streamline your visit and create a personalized plan of action for your needs. If you have an upcoming appointment, please make sure to fill out these forms and send these back to our office prior to your visit. They typically take 20-30 minutes to complete.
Insurance
At Foundation Dental Specialists, our first commitment is to your care. That means thoughtful, personal, specialist-level treatment without compromise. We want that care to be accessible, so we work with all PPO dental insurance plans. Our team will verify your benefits, handle the paperwork, and submit claims on your behalf so you receive the full benefit your plan allows. Because we are a specialty practice, we are not contracted with any individual insurance network, and we are unable to work with HMO plans.
It helps to know that dental insurance works differently than medical insurance. Rather than covering the full cost of care, dental plans contribute up to a fixed annual maximum regardless of which practice you visit. This contribution is a helpful benefit and we will make sure you receive it. Dental plans generally are not designed to cover the full cost of surgical or specialty treatment on their own.
Payment Methods & Healthcare Lenders
You will always know your investment before treatment begins, with no surprises. We accept all major credit cards (Visa, Mastercard, American Express, and Discover), along with cash, check, and HSA or FSA payments. We also offer flexible third party financing through Cherry, Sunbit, and CareCredit. Each option is built to fit a range of budgets, and our team is happy to walk you through them and help you find the approach that works best for you.
